A case of EVT for AAO of whom patient have PR3-ANCA positive
I report a case of 52 years old female. She was followed up with a small amount of diuretic as idiopathic edema. After bathing on October 31, 2018, she suddenly got right leg pain and cold sensation. She visited our hospital on November 2, her right foot looked pale that time. We can’t touch her pulse of DPA nor PTA. Her iliac artery, CFA, SFA and Popliteal artery were clear at CT angiography. We suspect her blood flow disturbance of distal part of her right tibial artery. And then We underwent an angiography. Her ATA and PTA were occluded and we tried intervention. Her occluded artery got reperfusion with Balloon expansion and she was released from pain and cold sensation. But her AAO relapsed soon. We tried reintervention immediately. The artery was occluded with recoil. I use balloon expansion and got successful reperfusion. And I avoid reocculusion to use a continuous artery injection of Alprostadil. My idea was successful. The other day I knew her PR3-ANCA was positive and tried to diagnose as any vasculitis. But I failed. I experienced AAO case with positive PR3-ANCA patient. We present it with literature consideration.
