Based on the approach for failure or accident study, the following two questions were investigated for the Fukushima accident. (1) Was the huge tsunami, induced by a huge earthquake, predictable at Fukushima-1 site? (2) If it was predictable, what preparations at Fukushima-1 could have reduced the severity of the accident? These questions were discussed within the study group, and its final report is described here.
