企画・趣旨のねらい

　今回の日本小児神経学会学術集会は第20回乳幼児けいれん研究会国際シンポジウムと合同で開催されるため，世界各国，特にアジア地域からの多数の参加者が見込まれている．この貴重な機会に，アジア各国での小児神経診療についての共通点や異なる点を議論し，小児神経診療の本質的な部分に迫りたい．本セッションでは，Phillinpines，Singapore，Turkeyから演者を招き，日本を含めた4か国におけるWest症候群の診療についての議論を企画した．West症候群は乳児期発症の代表的な難治性てんかんであるが，その病因は遺伝的素因から周産期脳障害まで多様である．また診療についても，治療として日本ではACTH注射薬，ビガバトリンが代表的だが，海外では副腎皮質ステロイド薬も使用されているなどの違いがあり，今回参加の4か国でも国ごとに異なっている可能性がある．これらのことから，West症候群は，疾患自体が多様性を持ち，かつ，各国共通の治療対象疾患という点で，今回の題材として最適と考えられる．West症候群を題材として病因・診断・治療についての個別の議論を行うとともに，それを通じて，小児神経診療において大切なことについての理解を共有したい．

　Since the 61st Annual Meeting of the Japanese Society of Child Neurology will be held jointly with the 20th Annual Meeting of Infantile Seizure Society, many participants from all over the world, especially the Asian region are expected.
　At this valuable opportunity, we would like to discuss common ground and differences and pursue essentials in child neurology.
　In this session, we invited the symposiasts from Phillinpines, Singapore and Turkey and organized discussions on the treatment of West syndrome.
　West syndrome is refractory infantile epilepsy observed around the world and its etiology is diverse from genetic predisposition to perinatal brain disorder.
　As for the treatment, ACTH injection, vitamin B6, and vigabatrin are representative in Japan, corticosteroid and ketogenic diet are also used.
　Based on these findings, West syndrome is thought to be optimal as the theme of this session in that the disease itself has diversity and it is common around the world.
　Discussion on etiology, diagnosis, and treatment on West syndrome as a theme, and through that, we would like to share understanding of what is important in child neurology.
