[MO-16] A Case of Endovascular Artery Repair with VIABAHN for the Anastomotic Pseudoaneurysm Rupture in the Left Common Carotid Artery after Total Arch Replacement.
A-84-year-old man was treated by total arch replacement(Triplex 28mm+Open STG 27mm+left subclavian artery repair: Triplex 8mm) for impending rupture of TAA. 3 month later, he had further examination for blood sputum, CTA showed huge hematoma around the left carotid artery anastomosis. We considered the bleeding and pseudoaneurysm at the anastomosis. Because of anatomical complex, we thought so difficult of device delivery from femoral site. Under general anesthesia, we performed cut down at the distal of left carotid artery. We inserted diagnostic catheter from femoral, and selective contrast of left carotid artery showed the blood jet at the distal site of anastomosis. We delivered GORE VIABAHN 9.0x50mm from distal site of left carotid artery, and deploy VIABAHN cross over rupture site. Angiography showed leakage from puncture site, so we added post dilatation by Mustang 9.0x40mm(8am), at last the leakage was stopped. We reported a case of endovascular artery repair with VIABAHN for the anastomotic pseudoaneurysm rupture in the left common carotid artery after total arch replacement.
