Novel predictor of dissection after balloon angioplasty in femoropopliteal artery stenosis, thickness of media “black band”

Background: Media of artery is expressed as black band between intima and adventitia by intravascular ultrasound (IVUS). Different from coronary artery, the thickness of media black band (tBB) of femoropopliteal (FP) artery vary considerably by individuals.

Method: Consecutive 78 FP lesions underwent balloon angioplasty without stent implantation with IVUS guide were enrolled. From pre-intervention IVUS image, cross section of reference and minimal lumen area were selected for analysis. Minimum and maximum tBB were measured manually (Fig) and compare with post-intervention result.

Result: At MLA, maximum and minimum tBB were 0.45mm (interquartile range[IQR]: 0.33 to 0.56) and 0.07mm ([IQR]: 0.07 to 0.17) respectively. There were no significant correlation with tBB and IVUS detected dissection angle. Sub analysis of severe stenotic lesions, lesions with percent plaque area >90%, revealed significant correlation between minimum tBB and dissection angle (r=0.50, p=0.004).

Conclusion: The thickness of media “black band” was a predictor of post-ballooning severe dissection in severe stenotic FP lesion.
