A 98-year-old woman performed transcatheter pacing system immediately after transcatheter aortic valve implantation.

Ukihiro Uehara

A 98-year-old woman suffered from dyspnea due to heart failure and severe aortic stenosis (AS). She presented with symptoms of dyspnea on exertion and leg edema (New York Heart Association class III). We decided to perform transfemoral TAVI by echocardiography revealed severe AS (aortic valve area, 0.45 cm2; maximum jet velocity, 3.97 m/s; and mean aortic pressure gradient, 33 mmHg). A femoral cutdown was performed to the both limbs, and the needle puncture into the artery to proceed the 23-mm EvolutR. EvolutR was then successfully deployed, however she developed complete atrio-ventricular block. Because she had high frailty score, we underwent transcatheter pacing system(TPS) (Micra) implantation continuously. Using a right femoral vein, TPS implantation was underwent without any troubles. The patient was able to walk on the next day and revealed good crinical course. We report acase which a 98-year-old woman performed transcatheter pacing system immediately after TAVI.
