The case of the shotgun stenting with two GORE VIABAHN VBX for the stenosis of terminalaorta.

A 72-year-old male with bilateral intermittent claudication(Rutherford-3) was referred to our hospital because of severe hypertension. His right ABI was 0.73, the left is 0.80. CTA showed severe stenosis with concentric plaque at the terminal aorta, but no lesion at the ostium-proximal of iliac artery. Previously at our hospital, we performed treatments with stent or stent-graft for similar cases. However, there were problems such as plaque protrusion, risk of distal protection, and surgical invasion with cut down. So we planned EVT with GORE VIABAHN VBX. The procedure was performed under local anesthesia. After inserting 7Fr sheathes in bilateral femoral arteries, we crossed 0.014 inch guide wire(Cruise) the lesion. After measuring the vessel diameter and length by using IVUS, we changed 0.014 inch guide wire to 0.035 inch guide wire(terumo Radifocus) and implant two VIABAHN VBX by shot-gun stenting. We report the case of the shotgun stenting with two GORE VIABAHN VBX for the stenosis of terminal aorta.
