The utility of a novel distal protection system with an OPTIMO guiding catheter for the occluded lesion of the iliac artery

A 67-year-old women was referred to our hospital with rest pain and ulcer of left toe. Her CT angiogram revealed that the left iliac artery and superficial femoral artery were occluded. We performed an EVT for the occluded lesion of the iliac artery. 5Fr guiding sheath was inserted from the antegrade fashion via the left brachial artery. An OPTIMO was inserted from the retrograde fashion via the left common femoral artery and inflated to prevent distal embolization. 6Fr sheath was inserted from the common femoral vein. The side-arm of the OPTIMO was connected to the side-arm of V sheath through a filter. The retrograde guidewire successfully passed through the CTO lesion. A balloon angioplasty and stent implantation was performed for the lesion. The final IVUS and angiogram showed good dilatation without huge thrombus. There were captured thrombotic material in the filter after the procedure.

This protection method is used during carotid artery stenting, that is Parodi method. This method reduces thromboembolic events because of the continuous blood circulation during distal protection. In addition, it could reduce the loss of blood.

This protection system is a good option of the EVT with a risk of thromboembolic event.
