~How toprevent a bleeding complication? ~

Kentaro Fukuda (Juntendo University Nerima Hospital)

【Objectives】The aim of this study was to review the effectiveness and complications of endovascular therapy (EVT) in our institute in 13 years’ experience.
【Methods】This study was retrospective, single-center, observational study of patients underwent EVT (n=633, 72.4 ± 9.3 years old, 79% male) admitted to our hospital from August 2005 to December 2017 inclusive. Patients were divided into three groups according to the era as followings; Group 1 (G1) is from August 2005 to December 2011, Group 2 (G2) from January 2012 to December 2014, Group 3 (G3) from January 2015 to December 2017. We compared Major Adverse Limb Event (MALE) within 30 days and bleeding complications defined according to Bleeding Academic Research Consortium (BARC) criteria in each era.
【Results】The number of patients with diabetes mellitus and hemodialysis were getting increase significantly. A success rate was consistent through the 13 years’ experience. Major Adverse Limb Event (MALE) rate in G3 most improved (G1; 10%, G2; 14%, and G3; 2%, p< 0.05), especially, a bleeding complication rate (G1; 16%, G2; 12%, G3; 3%, p <0.05).
【Conclusions】We found the improvement of MACE rate especially bleeding complication with the time. The reason may be the improvement of the skill of puncture using ultrasonography.
