[MP-1] A case of successful endovascular therapy for ALI after Forgaty catheter

Shoichiro Furukawa (Deparment of cardiology, Iizuka hospital)

ALI is a serious medical condition that requires rapid diagnosis and prompt initiation of appropriate treatment. In almost case, a surgical therapy is selected. Thrombectomy surgery, using Forgaty catheter, is effective and standard therapy for ALI. However, large amount of thrombus can sometimes be a problem, when a residual thrombus is remained. In case of a residual thrombus in popliteal (POP) artery to below the knee (BK) artery, additional treatment is turning point of decision. We would like to present a case of ALI treated with additional endovascular therapy (EVT) after Forgaty catheter using. A case is 69-year-old women with right leg rest pain. CT angiography showed thrombotic occlusion from right superficial femoral artery (SFA) to POP artery. There was not yet the anticoagulant therapy of atrial fibrillation. Since the time of onset was within 6 hours, she received a thrombectomy surgery. Although the value of creatine kinase (CK) was peaked after the surgery, her rest pain was left. We performed additional EVT for BK thrombotic lesions, which improved her symptom without any amputation or non-ambulatory. While also presenting other cases of ALI with additional EVT after thrombectomy surgery, we report a case of successful EVT for ALI after Forgaty catheter theraphy.
