[MP-11] Coiling for deep femoral artery of thigh hematoma after iliac stenting

Our case was a man in his seventies who had claudication. He had chronic atrial fibrillation and was given oral anticoagulation. His ABI was 0.76/0.64 and he had several stenosis in his iliac arteries. We performed endovascular treatment for these lesions. We put a sheath introducer at right CFA retrogradely and switch the sheath to a 6F guiding sheath. We treated both iliac stenosis with bare nitinol stents with the wire in his left deep femoral artery. We finished the EVT and made hemostasis. Several hours after the procedure he gradually felt pain and swelling on the left thigh. Emergent CTA indicated active bleeding at muscle perforator of deep femoral arteries. We decided to perform hemostasis. We advanced 0.014 wire with a microcatheter and confirmed a bleeding point using tip injection through the microcatheter. We had Figure 8-18 coils (the size was 2mm-5.0mm) in our room and put 3 those coils at the bleeding site. But we found another bleeding in a different branch. We put more 4 Tornado coils which was cut into adequite size. Finally, we were able to finish hemostasis. His anticoagulation and DFA wiring during the first procedure might be a cause of this bleeding.
