Aging leads to complicated changes in bladder and urethral activity. We have previously reported that aging was assumed to induce dysfunction in nitric oxide (NO)-mediated urethral smooth muscle relaxation with an increase of oxidative stress markers in the urethra, and that application of a NO donor and a stimulator of soluble guanylyl cyclase (sGC) could restore impaired urethral smooth muscle relaxation. Thus, age-associated ischemic and oxidative stress in the urethra might be correlated with impairment of the NO/sGC system and with coordination of the bladder and urethra. Now we are now ongoing to restore age-related irreversible changes in coordination of the bladder and urethra by low-intensity extracorporeal shock wave therapy (LiESWT). Furthermore, aging is associated with lower urinary tract symptoms, such as nocturia, resulting in increased health problems. We recently developed the digital self-health monitoring system of urine excretion (s-HMSU), as an alternative to frequency volume chart, in Kumejima Digital Health Project (KDHP). In this project, we ascertained that nocturia and shortened hours of undisturbed sleep could be signs of obesity/ metabolic syndrome. Now we are ongoing to develop app recording nocturia and shortened hours of undisturbed sleep with feedback. In this session, I will introduce what is going on to develop genesis of aging urethra and possibility of a sensing of nocturia as obesity/ metabolic syndrome?
