[PD-1-7] A case report of effects in the artwork on relieving pain in the older patient after surgical treatment of hip fracture

Introduction
Postoperative patients with musculoskeletal disorders often cause emotional distress due to postoperative pain. These patients may suffer depression and loss of independence if not treated properly. In this case study, we describe an occupational intervention for a patient who underwent surgical treatment for a hip fracture.
Patient Information
A 80s-year-old woman fractured her left hip in a fall accident. She was admitted to a rehabilitation ward in our hospital for recovery of Activities Daily of Living(ADL)after surgical treatment. She had previous history of included hip and knee replacement surgery, lumbar vertebrae fracture, and wrist joint deformity. She had a mild functional disability and needed her husband’s help.
Assessments
Occupational therapy was prescribed to recover ADL and return home. At the initial assessment, she had pain in her left leg (the Numerical Rating Scale for pain (NRS) 8-9 points). The functional independence Measure (FIM) score was 62 points (motor 28 points, cognition 34 points), and the Vitality Index (VI) score was 7 points. Seventeen days after surgery, her ADLs recovered up to supervised Levels. However, she still had pain in the left leg and other joints, such as her knees, shoulders, and fingers. (the NRS 6-7points). In addition, her motivation for rehabilitation and daily life decreased, and she often closed her eyes and mind, even during occupational therapy.
The patient’s main concerns and intervention
The case had emotional distress due to pains, which affected the motivation for rehabilitation and daily life. To deal with these problems. We used iron beads craft to relieve her emotional distress in occupational therapy. The activity with iron beads craft is useful in enhancing positive emotions through good-looking and colorful artwork.
Follow-up and Outcomes
In the beginning, she was not willing to do the artwork. However, after finishing the work with an occupational therapist, she was satisfied with the artwork. She created three works during her hospital stay and planned to give one of the works to her husband. Positive emotion influenced her attitude toward her rehabilitation. Before hospital discharge, the NRS of her hip pain decreased from 7 to 2 and she achieved her functional goal (the FIM 105 points: motor 71 points, cognitive 34 points). VI had improved from 7 points to 8 points, moreover the motivation for rehabilitation and daily life.
Discussion
Iron beads craft enhanced her positive emotion, resulting in relieving the stress of pain and improving her motivation for rehabilitation and daily life. It is implied that engagement in artwork with iron beads craft during hospital stays could be useful in patients who are suffering from postoperative pain.
