[II-P08-5-06] 先天性心疾患児におけるMyocardial Workと哺乳負荷心エコー

Background: We attempted to perform infant stress echocardiograms, which are those taken during and after a bottle feeding. Myocardial work (MCW), estimated using non-invasive pressure–strain loops. This study determined MCW using echocardiography during bottle feeding in infants with congenital heart disease (CHD).Methods: The study population comprised 29 infants with CHD. MCW consistent, global work index (GWI), and others were calculated using the EchoPAC. Echocardiographic measured during and after bottle feeding. Active feeding (AF) was defined as the start of feeding, End of feeding (EF) was defined as the time of completion of feeding. The infants were divided into two groups: those who needed invasive treatment in infancy (Ope) and those who needed follow-up (Follow). We compared the fractional change (FC%), defined as (AF minus EF)/EF.Results: In the Ope group, there were 7 large VSD and 1 pulmonary stenosis (PS) infant, and in the Follow group there were 19 small shunts VSD patients and 2 mild PS infants. The FC% in GWI was lower in the Ope group (median -21.6%) than in the Follow (12.9%) significantly, and in Ope VSD cases, FC in VSD peak velocity (11.0%) was lower than the Follow VSD (27.2%), significantly (p-value less than 0.01).Conclusions: Our findings indicate that stress echocardiography during bottle feeding might be a substitute for diastolic stress echocardiography in infants with CHD.
