[Aim]
Transitional medical care for patients with childhood-onset diseases is an important issue that has been raised at several academic conferences. However, discussions have mainly focused on how transitional medical care should be handled from the perspective of medical professionals, and there have been few opportunities to learn how patients and their family members consider such care and what they expect from it. For this seminar, I asked members of the Patient Family Association to create a poster entitled “What we expect from pediatricians in adult medical care.” I hope that this poster session will serve as an opportunity for you to learn of difficulties experienced by patients and their families and their expectations from pediatricians and to explore ways of applying such knowledge to daily medical practice.
