[P2-3] Restoring generalism: the new mission for family medicine
Dr Lee Kheng Hock is an Associate Professor of Family Medicine at the DukeNUS Medical School. He was the President of the College of Family Physicians Singapore from 2011 to 2017. He is currently a Council Member of the Academy of Medicine Singapore, the national body representing all specialists in Singapore.
In 2006, Dr Lee was invited to set up a clinical department of family medicine at the Singapore General Hospital, the largest tertiary hospital in Singapore. From 2006 to 2016, Dr Lee and his team experimented with different care models using the principles of family medicine and integrated care. The effectiveness of their care model was proven in 2 landmark RCT. Using this model of transitional care Dr Lee and his team of family physicians, nurse case managers and allied health professionals presently support more than 4000 elderly patients with complex care needs living in the community.
