The use of real world data (RWD) derived from electronic medical records (EHRs) is attracting attention. Primary and secondary use of EHR data is also discussed in the healthcare DX promoted by the Japanese government. Especially in the field of genomic medicine, how to obtain phenotypic data, or clinical data, is a high priority issue. Compared to genomic analysis, which is performed almost automatically after specimen collection, there is a strong need for methods to automate and streamline clinical data collection, which still relies on human labor. In this session, we will first show the FHIR applications in Japan in the fields of rare disease genomics, cancer genomics, and infectious diseases. These projects have created JASPEHR (the JApanese Standard Platform for EHRs) template profiles based on the FHIR Questionnaire and output the structured clinical data entered into the EHR as a vendor-neutral FHIR QuestionnaireResponse. Additionally, REDCap is also in the process of adapting the JASPEHR template and we will present the status of that. Next, as a global case study, we present HL7 Vulcan, one of the accelerator groups to promote HL7 FHIR. HL7 Vulcan was established in September 2019 by a group of representatives from government agencies, academic institutions, pharmaceutical companies, technology companies, standards development organizations, and other consortia, among others. We’ll introduce the purpose and concept of Vulcan to mature FHIR resources for research through use cases on secondary use of EHR data. Then, by presenting the projects in Vulcan, we expect to share with the audience HL7 Vulcan's vision for linking clinical and research through data interoperability.
