[I-P02-4-06] wire twist techniqueを用いた大腿血管アプローチによるPDA stent留置

IntroductionPatent ductus arteriosus (PDA) stenting has emerged as a viable alternative to surgical interventions for ductus arteriosus-dependent pulmonary circulation. Several approaches and techniques have been reported, but with sparse success.Herein, we report three PDA stenting cases to several types of PDA morphology from femoral vessel approach and introduce our technique with high success rate.Case PresentationCase 1 was a 3-day-old female with pulmonary atresia, an intact ventricular septum, and two stents implanted from the femoral artery (FA) to a PDA originating from the descending aorta. Case 2, an 11-day-old man with pulmonary atresia and ventricular septal defect (PA/VSD), was treated with stent implantation from the femoral vein to the PDA from the inferior aspect of the aortic arch. Case 3 was a 2-month-old female infant with PA/VSD and was implanted two stents to the PDA from innominate artery using FA approach. In all three interventions, a 0.014 micro guidewire was rotated and fixed in a peripheral PA to improve a stent tracking.ConclusionsA wire twist technique can be useful to implant PDA stents from femoral approach.
