[PR-7-4] Preliminary Experience with Endovascular Therapy for Buerger’s Disease

Objectives: The aim of this retrospective study was to clarify the clinical outcome of endovascular therapy for Buerger's disease(Thromboangiitis obliterans: TAO) patients. Methods: This study included a total of consecutive 12 lower or upper limbs in 12 denovo patients(mean age, 46±11 years) undergoing endovascular therapy between April 2007 and May 2014. Procedural success was defined as revascularization of at least one occlusive segment without 30% or more residual stenosis. Clinical success was defined as relief of ischemic pain, wound cure, avoidance of major amputation or improvement of claudication. Results: Clinical indications for revascularization were as follows; 6 instances of tissue loss in the lower extremity, 2 of tissue loss in the finger, and 4 calf claudication. Successful revascularization was achieved in all limbs. During a mean follow-up of 13±13 months(range, 1-48), clinical success was achieved in all cases, although 3 limbs required clinically driven repeat intervention for significant restenosis. There have been no deaths or major amputation. Conclusions: This preliminary experience with endovascular therapy produced a satisfactory clinical outcome in selected TAO patients. This novel approach might offer hope concerning the relief of pain, avoidance of major amputation, wound healing and improvement in quality of life.
