[P1-147] Ketogenic diet treatment in patients with mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes

[Objective] To study the clinical effect of the ketogenic diet treatment in patients with mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS)
[Methods] In the present study, retrospective analysis of ketogenic diet treatment in 2 cases of children with MELAS. We assessed the frequency of seizures, the level of lactic acid, the change of electroencephalogram (EEG) and magnetic resonance imaging (MRI) of brain, the changes in cognitive function and side effects after six months ketogenic diet treatment.
[Results] In case 1, during the ketogenic diet treatment, clinical symptoms such as headache and vomiting disappeared within one month, and frequency seizure disappears after 6 month. The high level of lactic acid was reduced gradually and was back to the normal range within one month. Brain MRI scan has no showed new lesions, and cognitive ability of patient got improved. In Case 2, frequency seizure disappears after ketogenic diet treatment. No side effects were observed in two cases.
[Conclusions] The ketogenic diet may lead to improvement of mitochondrial dysfunction in patients with MELAS, which promote better seizure control and less frequent stroke-like episodes.
