[P1-212] The Effects of Japanese Herbal Medicine, Goreisan, against Hypoxic–Ischemic Encephalopathy: Study in Childhood Rat Models

Regulation of secondary cerebral edema is of prognostic significance in hypoxic–ischemic encephalopathy (HIE). The traditional Japanese herbal medicine, Goreisan, effectively relieves brain edema in adults. However, its pharmacological mechanism and effectiveness in children are unknown. Aquaporin 4 (AQP4) plays an important role in cerebral edema pathogenesis. Therefore, we investigated the effects of Goreisan on HIE-associated brain edema and on AQP4 expression in childhood rats.
Twenty-six male juvenile Wistar rats (21 days old) were divided into Goreisan and control groups, who orally received Goreisan (2 g/kg) or saline, respectively. After introducing a transient embolism in the right middle cerebral artery, the right common carotid artery was ligated and dissected. Magnetic resonance imaging (MRI) was performed at 24 and 48 h after surgery, and survival rates were evaluated at 14 days post-surgery. In the Goreisan and control groups (n = 6 each), PCR measured AQP4 mRNA levels at 36 h after surgery.
MRI findings showed a significantly smaller affected brain area in the Goreisan group compared with the control group at 24 and 48 h after surgery. AQP4 mRNA expression was significantly suppressed in the affected and unaffected sides in the Goreisan group compared with controls. The survival rate was also significantly better in the Goreisan group than in the control group.
Goreisan suppresses brain edema in HIE and improves survival in childhood rats via regulation of AQP4 expression.
