[P1-47] Myanmar Experience on Infantile Spasms: A Case Series

Infantile spasms is a serious epilepsy syndrome. The semiology, electroencephalographic features and prognosis are different from other forms of epilepsies with different treatment. This is the first case series of infantile spasms in Myanmar.
Treatment naive children with infantile spasms aged 1 month to 2 years were included in the study. EEG was done on all patients and neuroimaging was done on patients without clinically identifiable etiology. All patients were treated with prednisolone 40mg/kg and the response was assessed on day 7. In patients with seizure control, the same dose of prednisolone was continued for another 1 week. If there is no seizure control, dose was increased to 60mg/day for another 1 week. Response was assessed on day 14 clinically and EEG.
Total 39 cases were enrolled over 2 years period. Males predominates females. Majority of the study population had age of onset at 3-7 months. More than half (54%) of patients had seizure frequency of 5-10 clusters per day. Sixty-seven percent had perinatal problems such as perinatal asphyxia, neonatal jaundice and neonatal meningitis. Developmental abnormality was found in 69%. Hypsarrhythmia was found on EEG in 25 patients, variants of hypsarrhythmia in 2 patients and other EEG patterns in 12 patients. Majority of patients (82%) responded to prednisolone.
We concluded that prednisolone is still cheap and effective treatment option for infantile spasms in resource poor settings like in Myanmar.
