[P1-55] A case of BECTS which exhibits electrical status epilepticus during sleep

Hideyo KINUGASA (Department of Pediatrics, Tsukuba Gakuen Hospital, Japan)

We report a case of benign childhood epilepsy with centrotemporal spikes (BECTS) which exhibits electrical status epilepticus during sleep(ESES). 7-year old boy had two episodes of nocturnal generalized tonic clonic seizures(GTC). Electroencephalogram(EEG) showed centrotemporal spikes, so valproate(VPA) treatment was started as diagnosis of BECTS. EEG findings gradually became worse. At age 8 years 11months, frequent diffuse S&W in awake, and electrical status epilepticus during sleep were observed. Also recurrent seizures occurred. Clobazam(CLB) or carbamazepine(CBZ) were tried but those were ineffective. When he was 9 years 2 months old, Levetiracetam(LEV) was added to VPA. After that, not only seizure frequency decreased, but also EEG findings were improved. Epileptic seizures were no longer seen after the age of 9 years 10 months. ESES disappeared at age 10 years 0 month, and EEG findings were normalized at age 11 years 3 months. In this case, LEV was effective for both seizure control and EEG improvement. We consider early introduction of treatment with LEV is recommended if EEG findings deteriorate during course of BECTS.
