[P2-109] A successful case of CSWS treated with corpus callosotomy

Sadao Nakamura (Department of Pediatrics, Ryukyu University, Japan)

[Introduction] Corpus callosotomy is effective against the drop attack. However, there is no certain opinion for surgical treatment for Continuous Spikes and Waves during Slow sleep (CSWS). We present a successful case of CSWS treated with corpus callosotomy.
[Case report] A Right-handed boy 5 years old, who had drug resistance epilepsy from 1 years and 4 months old. Although tonic seizures improved, drop attack and atypical absences frequently occurred. He had left spastic paresis and intellectual disability. A brain magnetic resonance imaging (MRI) of the brain showed region in right frontal, central and temporal cortex. Interictal electroencephalogram (EEG) recording during sleep showed general continuous spikes and waves. For hemispherotomy has a high risk, in order to improve the drop attack and confirm laterality, corpus callosotomy was performed at 5 years and 5 months. After surgery, the seizure disappeared and the right side EEG findings was also improved. Although the drug is followed only with valproic acid and a small amount of prednisolone (0.4mg/kg/day), there is no recurrence of seizure. In 2 years after surgery, language development has been confirmed, improving right side EEG findings was considered to be the cause. By cases, corpus callosotomy considered to be effective for CSWS.
