Wenhui Wang (Shanxi Children's Hospital, Taiyuan, Shanxi, China)

【Abstract】 Objective To investigate the clinical features of purulent meningitis caused by streptococcus pneumoniae. Methods Retrospectively analyze the clinical data of 26 children with purulent meningitis caused by Streptococcus pneumoniae in our hospital from January 2014 to February 2016. Results All of the cerebrospinal fluid (CSF) culture results of 26 cases were streptococcus pneumoniae. The age ranged from 2 months to 13 years old, of which less than 1 years old accounted for 38.5% (10/26), 65.4% under the age of 3 years old (17/26). All cases were with the symptom of fever, and associated with neurological involvement. The C-reactive protein(CRP), procalcitonin, erythrocyte sedimentation rate(ESR) and CSF protein increased significantly, CSF sugar decreased obviously. Cases whose CSF leukocyte significantly higher were accounted for 88.5%(23/26). 25 cases were eventually treatmented by vancomycin combinated with 3rd generations of cephalosporins antibiotics or other antibiotics. 7 cases were cured, 4cases improved markedly,in total 11cases had favorable prognosis;3 cases with sequelae and 12 cases died, the outcome of these 15cases was adverse. Conclusions We should pay more attention to the meningitis caused by streptococcus pneumoniae. Clinical suspicion of pneumonia streptococcus meningitis should be given the powerful therapy by vancomycin and other antibiotics as early as possible.
