[S40] Latest research on pharmacokinetics and safety evaluation for prediction of drug-induced kidney injury

Drug-induced kidney injury (DIKI) accounts for approximately one-fourth of clinically observed acute kidney injury, and its occurrence can affect dosing regimen and patient prognosis. To evaluate its risk in preclinical research, in vivo analysis using experimental animals is mainly performed. However, DIKI is often firstly observed in clinical trials and practice. The reason for its low predictability is species difference between humans and experimental animals in 1) drug distribution process to the kidney via drug transporters and drug-metabolizing enzymes, and 2) physiological responses of the kidney to DIKI-inducing factors such as drug reaction and hypoxia. Conventionally used renal cells hardly express these pharmacokinetic proteins and have low physiological responsiveness. To overcome the concern, it is important to construct in vitro evaluation system using human-derived cells and biomarkers which can predict clinical observation. In this symposium, we raise issues in the evaluation of pharmacokinetics in the kidney and DIKI in drug discovery and development in pharma industry. Moreover, we introduce novel approaches about cell culture systems for DIKI and biomarkers related to the progression to chronic lesion from DIKI.
