With the coming of a rapidly aging society with numerous deaths and a declining birthrate, Japan's medical system is called upon to offer care tailored to each individual's life stages, from birth to death.
In 2015, the "Vision of Pharmacies for Patients" was proposed as part of preparing the ground in response to the rapidly aging society for the community-based comprehensive care that will begin in 2025.The core of this vision is the primary care role of pharmacies, and it also explicitly includes outpatient and home healthcare functions as well as health support functions. In particular, 24-hour home care support is mentioned, and pharmacies are clearly presented as medical care providers.
Pharmacists are, by law, called upon to, contribute to the improvement and promotion of public health and thereby ensure healthy lives for the general population, and are therefore seamlessly involved in securing the health of patients. This session will provide an opportunity for them to consider their own work from the perspective of the primary care principles of "accessibility," "coordination," "comprehensiveness," "continuity," and "accountability".
